How to Choose a Home Care Agency in Massachusetts: Twelve Questions to Ask

Most home care agencies describe themselves in nearly identical language. Compassionate, trusted, family owned. None of that tells you whether an aide will show up on Thursday.
These twelve questions do. Ask them on the phone before the assessment and pay attention to how quickly and specifically they are answered.
Questions about the caregivers
The people entering the home are the service. Everything else is logistics.
- Are caregivers employees of the agency, or independent contractors? Employees are supervised, insured, and covered by workers compensation
- What background checks and reference verification do you run, and how often are they repeated?
- What training is required before an aide works alone, and what continuing training do you provide?
- Will the same aide come consistently, and who covers a call out?
Questions about supervision
Supervision is where agencies quietly differ the most.
- Which nurse writes and reviews the plan of care, and how often does she visit the home?
- How are aide notes documented and how can the family see updates?
- What is the process if we want a different caregiver? Ask for the actual process, not reassurance
Questions about reliability and cost
Reliability is the thing families complain about most after signing.
- Who answers the phone at 9 pm on a Sunday, and is it a person or a service?
- What is your response time when a shift is missed?
- What exactly will we owe, and what happens if authorization changes mid month?
- Do you handle the insurance submission and follow up, or is that our responsibility?
- Can you serve our specific town on the schedule we need? Coverage in Worcester is not coverage in Athol
Warning signs
A few responses should end the conversation regardless of how warm the sales call felt.
- Promising insurance approval or a specific number of hours before any assessment
- Pressure to sign the same day or a long contract with a difficult exit
- Vagueness about who supervises the clinical plan
- No clear after hours contact
- Reluctance to put pricing and cancellation terms in writing
- Claiming affiliation with or endorsement by a government program or health plan
What a good first conversation feels like
You should hang up knowing the name of the person coordinating your case, what happens next, and roughly when. Nobody should have promised an outcome that depends on a clinical assessment they have not yet done.
A good agency will also tell you when they are not the right fit. If round the clock skilled care is required and the agency cannot staff it in your town, saying so is a sign of competence rather than a lost sale.
